Case Management support is provided to adults with disabilities to coordinate their overall services through an individualized planning process. Case Management staff offers individuals opportunities to develop personal relationships, to have positive roles in community life and to gain increased control of their lives.
Case Management staff advocate for integration of individuals with developmental disabilities into the mainstream of their communities. Supports for individuals are facilitated through Seguin and by linking them with various services, agencies and organizations in each community.
The Case Manager assures that individual's rights are protected, and that the services and supports promote individual choice and match with specific needs and personal preferences identified. The Case Manager also encourages each person to discover and identify hopes, dreams and visions of the future. Once this is achieved, the individual, with assistance as needed from the Case Manager and support team, identifies goals for the future and creates an action plan to accomplish those goals.
The ultimate goal for each individual is to have the opportunity to become a contributing member of the community, and to have the supports needed to fulfill hopes and dreams in all areas of life.
